Il repr\u00e9sente une colonie complexe, organis\u00e9e et multi-esp\u00e8ces de micro-organismes int\u00e9gr\u00e9e dans une matrice polysaccharidique protectrice.\" data-link=\"https:\/\/wiki.voka.io\/fr\/glossaire\/plaque-dentaire\/\">biofilm<\/span> appel\u00e9 plaque dentaire. Les l\u00e9sions carieuses se d\u00e9veloppent dans les zones o\u00f9 la plaque dentaire s\u2019accumule.<\/p>\n<p>La consommation fr\u00e9quente et prolong\u00e9e de glucides facilement fermentescibles augmente la pr\u00e9sence de flore cariog\u00e8ne dans le biofilm. Ces micro-organismes sont capables de rapidement m\u00e9taboliser les glucides alimentaires pour former des acides organiques, ainsi que de survivre dans des conditions de faible pH. Il s\u2019agit principalement de <em>Streptococcus mutans<\/em> et <em>Lactobacilli<\/em>, mais aussi de <em>Streptococcus sobrinus<\/em>, <em>Streptococcus oralis, Actinomyces israelii, Actinomyces gerencseriae, Bifidobacteriaet Prevotella<\/em>. Elle est situ\u00e9e sous l\u2019\u00e9mail dans la r\u00e9gion de la couronne et sous le c\u00e9ment dans la zone racinaire, entourant la chambre pulpaire et les canaux radiculaires. En termes de propri\u00e9t\u00e9s physiques, la dentine est plus r\u00e9sistante que l\u2019os mais plus \u00e9lastique que l\u2019\u00e9mail.\" data-link=\"https:\/\/wiki.voka.io\/fr\/glossaire\/dentine\/\">dentine<\/span> sont en \u00e9quilibre. <\/p>\n<p>Le m\u00e9tabolisme des glucides alimentaires (glucose, fructose, saccharose, maltose, etc.) par les bact\u00e9ries entra\u00eene une accumulation d\u2019acides et une diminution du pH. <\/p>\n<p>Lorsque le pH diminue suffisamment (5,2\u20135,5), les min\u00e9raux des tissus durs de la dent commencent \u00e0 se dissoudre. Cette perte progressive de min\u00e9raux lors de leur dissolution par l\u2019acide (d\u00e9min\u00e9ralisation) est au centre du processus de d\u00e9veloppement de la carie dentaire.<\/p>\n<p>Plus les glucides p\u00e9n\u00e8trent dans la cavit\u00e9 orale avec les aliments et plus les r\u00e9sidus alimentaires restent longtemps \u00e0 la surface des dents, plus les bact\u00e9ries produisent de l\u2019acide, le pH diminue et la d\u00e9min\u00e9ralisation de l\u2019\u00e9mail progresse. Lorsque les bact\u00e9ries atteignent la dentine, en plus de continuer \u00e0 produire de l\u2019acide, elles commencent \u00e0 exercer une activit\u00e9 prot\u00e9olytique. En cons\u00e9quence, le composant organique (collag\u00e8ne) de la dentine est d\u00e9truit. <\/p>\n<p>La progression des caries peut \u00eatre ralentie ou arr\u00eat\u00e9e par la r\u00e9duction de la fr\u00e9quence et de la dur\u00e9e des attaques acides, par exemple en am\u00e9liorant l\u2019hygi\u00e8ne ou en limitant la consommation de sucre. Les compos\u00e9s fluor\u00e9s (fluorure) favorisent les processus de remin\u00e9ralisation et retardent la d\u00e9min\u00e9ralisation, r\u00e9duisent la solubilit\u00e9 de l\u2019\u00e9mail et augmentent sa r\u00e9sistance aux attaques acides.<\/p>\n<h2 class=\"wp-block-heading\" id=\"classification-des-caries\"><strong>Classification des caries<\/strong><\/h2>\n<h3 class=\"wp-block-heading\" id=\"anatomie\"><strong>Anatomie<\/strong><\/h2>\n<p>Les l\u00e9sions carieuses apparaissent toujours sur la surface de la dent qui est en contact direct avec la cavit\u00e9 orale : surface de l\u2019\u00e9mail, c\u00e9ment radiculaire expos\u00e9, dentine expos\u00e9e. Les caries se d\u00e9veloppent le plus souvent dans les endroits qui accumulent de la plaque dentaire : dans les puits et les fissures, sur les faces lat\u00e9rales des dents, au niveau du collet. En cas de processus carieux actif et d\u2019hygi\u00e8ne insuffisante, les caries peuvent affecter les \u00ab zones immunitaires \u00bb : les surfaces lisses, l\u2019\u00e9quateur et les cuspides de la dent.<\/p>\n<p>Carie de l\u2019\u00e9mail : au stade initial (E1), apr\u00e8s l\u2019\u00e9limination de la plaque et le s\u00e9chage de la dent, l\u2019\u00e9mail appara\u00eet macroscopiquement intact, avec une tache blanche mate indiquant un foyer de d\u00e9min\u00e9ralisation. \u00c0 un stade ult\u00e9rieur (E2), la tache blanche est d\u00e9j\u00e0 visible sur la surface humide de l\u2019\u00e9mail. Cette tache peut \u00eatre color\u00e9e en diff\u00e9rentes nuances de brun avec des colorants alimentaires. \u00c0 l\u2019int\u00e9rieur de l\u2019\u00e9mail, la l\u00e9sion a l\u2019apparence d\u2019un c\u00f4ne dont le sommet pointe vers la jonction \u00e9mail-dentine (JED). Lorsque le processus atteint la JED et p\u00e9n\u00e8tre dans la dentine, la l\u00e9sion continue de se propager le long des tubules dentinaires.<\/p>\n<p>Carie de la dentine : dans la zone du d\u00e9faut, l\u2019\u00e9mail d\u00e9min\u00e9ralis\u00e9 peut rester intact (\u00e9mail pseudo-intact), emp\u00eachant les bact\u00e9ries de p\u00e9n\u00e9trer dans la zone de d\u00e9min\u00e9ralisation. Macroscopiquement, ce d\u00e9faut appara\u00eet comme une fissure ou une tache pigment\u00e9e \u00e0 la surface de l\u2019\u00e9mail, sous laquelle on peut discerner les contours plus fonc\u00e9s de la dentine affect\u00e9e. <\/p>\n<p>Lorsque l\u2019\u00e9mail perd environ 30 \u00e0 40 % de ses min\u00e9raux dans la zone de la l\u00e9sion, il devient si poreux et si fragile qu\u2019il peut facilement se briser et former un d\u00e9faut ressemblant \u00e0 une cavit\u00e9. Les bact\u00e9ries s\u2019engouffrent alors dans la profondeur du d\u00e9faut. Sous l\u2019influence de prot\u00e9ases bact\u00e9riennes, la dentine d\u00e9min\u00e9ralis\u00e9e se n\u00e9crose (zone de p\u00e9n\u00e9tration bact\u00e9rienne), et la zone de d\u00e9min\u00e9ralisation s\u2019\u00e9tend plus profond\u00e9ment vers la pulpe. Autour de la zone de d\u00e9min\u00e9ralisation, une scl\u00e9rose r\u00e9actionnelle de la dentine se produit. Les tiers externe (D1), moyen (D2) et interne (D3) de la dentine sont ensuite successivement touch\u00e9s. Macroscopiquement, la l\u00e9sion active se pr\u00e9sente comme une cavit\u00e9 remplie d\u2019une dentine n\u00e9cros\u00e9e, molle, gluante, brun-jaun\u00e2tre, qui s\u2019enl\u00e8ve facilement par grattage. La zone superficielle se propage le long des prismes d\u2019\u00e9mail vers la JED, puis le long des tubules dentinaires. La r\u00e9action au stimulus thermique dispara\u00eet imm\u00e9diatement ou quelques secondes apr\u00e8s son arr\u00eat ;<\/li>\n<li>Transillumination par fibre optique (FOTI) : transillumination des dents \u00e0 travers le point de contact. La l\u00e9sion carieuse appara\u00eet comme une tache sombre ;<\/li>\n<li>Fluorescence quantitative au laser : expos\u00e9s \u00e0 la lumi\u00e8re du laser, les tissus dentaires sains et cari\u00e9s deviennent fluorescents en \u00e9mettant des ondes lumineuses de diff\u00e9rentes longueurs ;<\/li>\n<li>Mesure de l\u2019imp\u00e9dance \u00e9lectrique : les tissus d\u00e9min\u00e9ralis\u00e9s ont une r\u00e9sistance \u00e9lectrique inf\u00e9rieure \u00e0 celle des tissus sains (cette m\u00e9thode est d\u2019un usage limit\u00e9 sur les faces occlusales) ;<\/li>\n<li>S\u00e9paration temporaire des dents \u00e0 l\u2019aide d\u2019anneaux en caoutchouc : permet de visualiser des l\u00e9sions interproximales ;<\/li>\n<li>Coloration : l\u2019\u00e9mail d\u00e9min\u00e9ralis\u00e9 et le collag\u00e8ne d\u00e9natur\u00e9 de la dentine absorbent le colorant, contrairement aux tissus sains.<\/li>\n<\/ul>\n<h2 class=\"wp-block-heading\" id=\"manifestations-cliniques\"><strong>Manifestations cliniques<\/strong><\/h2>\n<p>Les caries de l\u2019\u00e9mail (E1, E2) ne provoquent pas de plaintes, seul un d\u00e9faut esth\u00e9tique peut g\u00eaner les patients. Sur le plan clinique, une tache blanche ou pigment\u00e9e sur l\u2019\u00e9mail est constat\u00e9e.<\/p>\n<p>En cas de caries de la dentine (D1\u2013D3), caries du c\u00e9ment ou caries secondaires, les patients n\u2019expriment parfois aucune plainte. Ils peuvent \u00e9voquer la pr\u00e9sence d\u2019une cavit\u00e9 dans la dent ou d\u2019un d\u00e9faut esth\u00e9tique, une douleur dentaire localis\u00e9e de courte dur\u00e9e induite par des stimuli chimiques, thermiques ou m\u00e9caniques, des r\u00e9sidus alimentaires coll\u00e9s sur les dents ou des difficult\u00e9s \u00e0 utiliser le fil dentaire. Cliniquement, on constate la pigmentation du puits ou de la fissure, des bords de l\u2019ancienne restauration, le coincement de la sonde dans cette zone (classe I selon Black, carie secondaire), une
