Taxonomiquement, la famille <em>Herpesviridae<\/em> comprend trois sous-familles (<em>Alphaherpesvirinae<\/em>, <em>Betaherpesvirinae<\/em> et <em>Gammaherpesvirinae<\/em>). Plus de 100\u00a0virus herp\u00e8s ont \u00e9t\u00e9 identifi\u00e9s chez diverses esp\u00e8ces de vert\u00e9br\u00e9s, dont 9 sont prouv\u00e9s comme infectant les humains. <\/p>\n<p class=\"wp-block-paragraph\">Ils causent une large gamme de maladies, allant de l\u00e9sions mucocutan\u00e9es (oropharynx, r\u00e9gion g\u00e9nitale) \u00e0 des conditions s\u00e9v\u00e8res et invalidantes (k\u00e9ratite, enc\u00e9phalite, infections cong\u00e9nitales et n\u00e9oplasies malignes associ\u00e9es aux virus). Les individus infect\u00e9s servent de r\u00e9servoirs pour les virus herp\u00e8s et sont des sources d\u2019infection pour des contacts potentiels tout au long de leur vie.<\/p>\n<h2 class=\"wp-block-heading\" id=\"epidemiologie-des-virus-herpes\">\u00c9pid\u00e9miologie des virus herp\u00e8s<\/h2>\n<p class=\"wp-block-paragraph\">D\u2019un point de vue \u00e9pid\u00e9miologique, les virus herp\u00e8s sont les agents pathog\u00e8nes humains les plus largement r\u00e9pandus, et le taux de s\u00e9ropr\u00e9valence varie selon les pays en fonction de l\u2019\u00e2ge et des facteurs g\u00e9ographiques et socio-\u00e9conomiques. Une partie significative de la population est infect\u00e9e par au moins un ou plusieurs virus herp\u00e8s humains (HHV), souvent de mani\u00e8re asymptomatique. <\/p>\n<p class=\"wp-block-paragraph\">Environ 3,8\u00a0milliards de personnes sont infect\u00e9es par le HSV-1 et environ 520\u00a0millions par le HSV-2. Le virus Epstein-Barr (EBV) et le virus varicelle-zona (VZV) se retrouvent chez plus de 90\u00a0% de la population adulte mondiale, et le cytom\u00e9galovirus (CMV) dans 40 \u00e0 100\u00a0%. Des anticorps contre les HHV-6 et HHV-7 sont d\u00e9tect\u00e9s chez un grand nombre d\u2019enfants d\u00e8s l\u2019\u00e2ge de 5\u00a0ans. <\/p>\n<p class=\"wp-block-paragraph\">Le virus herp\u00e8s humain de type 8 (KSHV, HHV-8) est associ\u00e9 au sarcome de Kaposi chez les patients atteints d\u2019infection par le VIH au stade du SIDA, certaines tumeurs solides et des maladies lymphoprolif\u00e9ratives. Les cas d\u2019infections humaines au HHV ne pr\u00e9sentent pas de saisonnalit\u00e9 (le contact \u00e9troit se produit toute l\u2019ann\u00e9e).<strong><\/strong><\/p>\n<h2 class=\"wp-block-heading\" id=\"transmission-des-virus-herpes\">Transmission des virus herp\u00e8s<\/h2>\n<p class=\"wp-block-paragraph\">Les HHV sont transmis par diverses voies selon le type\u00a0:<\/p>\n<ol class=\"wp-block-list\">\n<li>Transmission par contact (direct)\u00a0: via les fluides biologiques lors du contact direct avec les membranes muqueuses et la peau\u00a0:\n<ul class=\"wp-block-list\">\n<li>Salive\u00a0: tous les membres de la famille\u00a0;<\/li>\n<li>Contenu v\u00e9siculaire\u00a0: alpha-virus herp\u00e8s\u00a0;<\/li>\n<li>S\u00e9cr\u00e9tions g\u00e9nitales\u00a0: HSV, CMV.<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<p class=\"wp-block-paragraph\">Bien que les HHV soient hautement infectieux apr\u00e8s inoculation dans les membranes muqueuses ou la peau, leurs virions sont instables en dehors des cellules humaines, et la plupart des transmissions n\u00e9cessitent un contact interpersonnel direct et \u00e9troit (muqueuse \u00e0 muqueuse non infect\u00e9e ou peau lors de contacts intimes\u00a0: baiser ou contact sexuel). Bien que la peau soit moins sujette \u00e0 l\u2019inoculation directe, la probabilit\u00e9 d\u2019infection augmente en cas de br\u00e8ches dans son int\u00e9grit\u00e9 (surfaces br\u00fbl\u00e9es, ecz\u00e9ma, dermatite atopique, herp\u00e8s gladiatorum chez les lutteurs, ou herp\u00e8s rugbiorum chez les joueurs de rugby). Les virus peuvent persister pendant plusieurs heures sur les surfaces et les objets, mais un tel contact indirect n\u2019est pas associ\u00e9 au risque de propagation des HHV.<\/p>\n<ol start=\"2\" class=\"wp-block-list\">\n<li>Par voie a\u00e9rienne (a\u00e9rosol)\u00a0: VZV.<\/li>\n<li>Transmission de la m\u00e8re \u00e0 l\u2019enfant\u00a0: intra-ut\u00e9rine, intrapartum (HSV, VZV, CMV).<\/li>\n<\/ol>\n<h2 class=\"wp-block-heading\" id=\"proprietes-biologiques-generales-des-virus-herpes\">Propri\u00e9t\u00e9s biologiques g\u00e9n\u00e9rales des virus herp\u00e8s<\/h2>\n<p class=\"wp-block-paragraph\">Tous les virus herp\u00e8s codent des enzymes uniques impliqu\u00e9es dans la synth\u00e8se de l\u2019ADN viral et offrent des cibles uniques pour l\u2019inhibition antivirale. La r\u00e9plication (du latin replicatio \u2013 renouvellement, synth\u00e8se et assemblage de l\u2019ADN viral, formation de la capside) se produit dans les noyaux des cellules humaines.<\/p>\n<p class=\"wp-block-paragraph\">Une caract\u00e9ristique distinctive importante de tous les virus herp\u00e8s humains (HHV) est leur capacit\u00e9 \u00e0 d\u00e9velopper des infections latentes suite \u00e0 l\u2019infection primaire (ils ne sont pas \u00e9limin\u00e9s apr\u00e8s l\u2019infection primaire\u00a0; le g\u00e9nome viral persiste dans certains types de cellules (cibles), sp\u00e9cifiques \u00e0 chaque virus \u2013 tropisme tissulaire unique). Les individus infect\u00e9s restent des r\u00e9servoirs de virus tout au long de leur vie et sont des sources de virus pour les contacts sensibles.<\/p>\n<p class=\"wp-block-paragraph\">Latence (du latin latentis \u2013 cach\u00e9, invisible). Pendant la latence, les g\u00e9nomes viraux sont maintenus de mani\u00e8re extrachromosomique ou int\u00e9gr\u00e9s dans l\u2019ADN de la cellule. La r\u00e9activation depuis l\u2019\u00e9tat de latence se produit en r\u00e9ponse \u00e0 divers irritants ou stimuli (stress, inflammation, immunosuppression, menstruation, exposition au rayonnement ultraviolet\/froid, etc.), permettant la propagation du virus parmi les individus non immunis\u00e9s (sensibles). La r\u00e9activation est principalement asymptomatique, bien qu\u2019elle puisse entra\u00eener une rechute symptomatique de la maladie, jusqu\u2019\u00e0 des conditions mena\u00e7ant le pronostic vital (principalement chez les personnes immunod\u00e9prim\u00e9es).<\/p>\n<p class=\"wp-block-paragraph\">Les HHV utilisent des strat\u00e9gies \u00e0 plusieurs niveaux pour \u00e9chapper aux r\u00e9ponses immunitaires ciblant l\u2019immunit\u00e9 inn\u00e9e et adaptative, leur permettant d\u2019\u00e9tablir des infections stables et \u00e0 vie. Les composants cl\u00e9s de la structure comprennent\u00a0:<\/p>\n<ul class=\"wp-block-list\">\n<li><strong>G\u00e9nome ADN<\/strong>\u00a0: situ\u00e9 au centre, il s\u2019agit d\u2019une grande mol\u00e9cule lin\u00e9aire bicat\u00e9naire.<\/li>\n<li><strong>Capside<\/strong>\u00a0: une coque icosa\u00e9drique constitu\u00e9e de capsom\u00e8res, enfermant le g\u00e9nome. <\/li>\n<li><strong>T\u00e9gument<\/strong>\u00a0: une couche amorphe entourant la capside. Il contient les facteurs viraux essentiels pour moduler l\u2019immunit\u00e9 et le transport intracellulaire. <\/li>\n<li><strong>Enveloppe lipidique bicouche<\/strong>\u00a0: la couche la plus ext\u00e9rieure, form\u00e9e \u00e0 partir des membranes des cellules de l\u2019h\u00f4te. La r\u00e9activation des b\u00e9ta-virus herp\u00e8s cause des maladies chez les individus immunod\u00e9prim\u00e9s.<\/li>\n<li><strong>Gamma-virus herp\u00e8s (EBV, KSHV\/HHV-8)<\/strong>. Infectent principalement les cellules lympho\u00efdes. L\u2019infection persistante est \u00e9troitement associ\u00e9e aux troubles lymphoprolif\u00e9ratifs et aux n\u00e9oplasmes malins.<\/li>\n<\/ol>\n<h2 class=\"wp-block-heading\" id=\"methodes-de-diagnostic-des-virus-herpes\">M\u00e9thodes de diagnostic des virus herp\u00e8s<\/h2>\n<p class=\"wp-block-paragraph\">Les m\u00e9thodes de diagnostic initial reposent cliniquement sur des modifications caract\u00e9ristiques (l\u00e9sions sur la peau et\/ou les muqueuses).<\/p>\n<h3 class=\"wp-block-heading\" id=\"diagnostic-en-laboratoire\">Diagnostic en laboratoire<\/h3>\n<ul class=\"wp-block-list\">\n<li>Culture cellulaire (diagnostic des alpha-virus herp\u00e8s \u00e0 travers des cycles de r\u00e9plication courts et des effets cytopathiques)\u00a0;<\/li>\n<li>R\u00e9action d\u2019immunofluorescence (diagnostic du HHV-1 et du HHV-2)\u00a0;<\/li>\n<li>PCR (d\u00e9tection de l\u2019ADN, tout milieu biologique)\u00a0;<\/li>\n<li>Analyse s\u00e9rologique (souvent une v\u00e9rification r\u00e9trospective de l\u2019infection).<\/li>\n<\/ul>\n<figure class=\"wp-block-image size-large\"><img decoding=\"async\" src=\"https:\/\/storage.googleapis.com\/dev_wiki_voka_io_303011\/articles\/en\/infectious-diseases\/herpesviruses\/laboratory-diagnosis-of-human-herpesvirus-tests-and-methods.webp\" alt=\"Diagnostic en laboratoire du virus herp\u00e8s humain\"><figcaption class=\"wp-element-caption\">Diagnostic en laboratoire du virus herp\u00e8s humain<\/figcaption><\/figure>\n<h2 class=\"wp-block-heading\" id=\"prevention\">Pr\u00e9vention<\/h2>\n<p class=\"wp-block-paragraph\">Mesures individuelles de pr\u00e9vention de l\u2019infection\u00a0:<\/p>\n<ol class=\"wp-block-list\">\n<li><strong>Prophylaxie vaccinale<\/strong>. Il n\u2019existe actuellement pas de vaccins homologu\u00e9s contre le HHV-1 ou le HHV-2\u00a0; seuls des vaccins pour pr\u00e9venir l\u2019infection par le VZV sont disponibles (vivant att\u00e9nu\u00e9 pour la varicelle et recombinant pour le zona).<\/li>\n<li><strong>Immunoprophylaxie<\/strong>.
