{"id":7204,"date":"2026-06-01T13:04:08","date_gmt":"2026-06-01T10:04:08","guid":{"rendered":"https:\/\/wiki.voka.io\/glossary\/bulbaersyndrom\/"},"modified":"2026-06-25T12:13:48","modified_gmt":"2026-06-25T09:13:48","slug":"bulbaersyndrom","status":"publish","type":"glossary_item","link":"https:\/\/wiki.voka.io\/de\/glossar\/bulbaersyndrom\/","title":{"rendered":"Bulb\u00e4rsyndrom"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">Bulb\u00e4rsyndrom (vom lat. <em>bulbus<\/em> \u2014 Zwiebel) ist ein schwerwiegender neurologischer Symptomenkomplex, der auf einer fokalen L\u00e4sion des unteren Abschnitts des Hirnstamms beruht. <\/p>\n\n<p class=\"wp-block-paragraph\">Die Pathologie tritt bei Sch\u00e4digung der Kerne oder Wurzeln dreier Hirnnerven gleichzeitig auf: N. glossopharyngeus, N. vagus und N. hypoglossus.<\/p>\n\n<h2 class=\"wp-block-heading\" id=\"h-&#x44D;&#x442;&#x438;&#x43E;&#x43B;&#x43E;&#x433;&#x438;&#x44F;-&#x438;-&#x43F;&#x430;&#x442;&#x43E;&#x444;&#x438;&#x437;&#x438;&#x43E;&#x43B;&#x43E;&#x433;&#x438;&#x44F;\"><strong>\u00c4tiologie und Pathophysiologie<\/strong><\/h2>\n\n<p class=\"wp-block-paragraph\">Die Medulla oblongata ist ein lebenswichtiges Zentrum, das das Gehirn mit dem R\u00fcckenmark verbindet. Bei schweren Sch\u00e4del-Hirn-Traumata ist seine Sch\u00e4digung selten prim\u00e4r. Am h\u00e4ufigsten sind die Stammstrukturen sekund\u00e4r betroffen \u2014 sie werden grob komprimiert und im gro\u00dfen Hinterhauptsloch des Sch\u00e4dels eingeklemmt, wenn das \u00d6dem der Hemisph\u00e4ren zunimmt (Einklemmungsprozess).<\/p>\n\n<p class=\"wp-block-paragraph\">Kompression oder Isch\u00e4mie der motorischen Kerne der 9., 10. und 12. Paare der Hirnnerven f\u00fchrt zur Entwicklung einer schlaffen (peripheren) L\u00e4hmung der Muskeln des Rachens, des Kehlkopfs, des weichen Gaumens und der Zunge. Der pathologische Prozess zerst\u00f6rt vollst\u00e4ndig den komplexen reflexartigen Akt des Schluckens und der Stimmgebung.<\/p>\n\n<h2 class=\"wp-block-heading\" id=\"h-&#x43A;&#x43B;&#x438;&#x43D;&#x438;&#x447;&#x435;&#x441;&#x43A;&#x43E;&#x435;-&#x437;&#x43D;&#x430;&#x447;&#x435;&#x43D;&#x438;&#x435;\"><strong>Klinische Signifikanz<\/strong><\/h2>\n\n<p class=\"wp-block-paragraph\">Das Syndrom manifestiert sich durch eine klassische und \u00e4u\u00dferst gef\u00e4hrliche Triade von Symptomen. Es tritt Dysphagie (vollst\u00e4ndige Unf\u00e4higkeit, Nahrung oder Speichel zu schlucken), Dysarthrie (unverstehbare, verschwommene Sprache) und Dysphonie (Heiserkeit oder Stimmverlust) auf. Bei der Untersuchung wird ein Fehlen des Pharyngealreflexes und ein Herabh\u00e4ngen des weichen Gaumens festgestellt.<\/p>\n\n<p class=\"wp-block-paragraph\">Dieser Zustand stellt eine unmittelbare Bedrohung f\u00fcr das Leben des Patienten dar. Aufgrund der L\u00e4hmung der Rachen- und Epiglottismuskulatur gelangen Speichel und Nahrung ungehindert in die Atemwege (Aspiration), was unweigerlich zu Erstickung oder Entwicklung schwerster Aspirationspneumonie f\u00fchrt. Zudem befinden sich in der N\u00e4he dieser Kerne Atem- und vasomotorische Zentren, deren Kompression Herz- und Atemstillstand verursacht.
