In der Neurotraumatologie ist dies eines der wichtigsten lokalen Symptome der Gehirnkompression.<\/p>\n\n<h2 class=\"wp-block-heading\" id=\"h-&#x44D;&#x442;&#x438;&#x43E;&#x43B;&#x43E;&#x433;&#x438;&#x44F;-&#x438;-&#x43F;&#x430;&#x442;&#x43E;&#x444;&#x438;&#x437;&#x438;&#x43E;&#x43B;&#x43E;&#x433;&#x438;&#x44F;\"><strong>\u00c4tiologie und Pathophysiologie<\/strong><\/h2>\n\n<p class=\"wp-block-paragraph\">Die Gr\u00f6\u00dfe der Pupille wird durch zwei Muskeln reguliert: den Schlie\u00dfmuskel (zur Verengung) und den Dilatator (zur Erweiterung). Die Funktion des Schlie\u00dfmuskels wird von parasympathischen Nervenfasern gesteuert, die im Augenbewegungsnerv (drittes Hirnnervenpaar) verlaufen.<\/p>\n\n<p class=\"wp-block-paragraph\">Eine Besonderheit der Anatomie ist, dass sich diese empfindlichen Fasern an der Oberfl\u00e4che des Nervenstamms befinden. Wenn bei einer temporalen Einlagerung verschobenes Hirngewebe beginnt, auf den Augenbewegungsnerv zu dr\u00fccken, werden diese oberfl\u00e4chlichen Fasern als erste in Mitleidenschaft gezogen. Das Signal zur Pupillenverengung wird blockiert \u2014 und der Dilatator erh\u00f6ht ungehindert die Pupille.<\/p>\n\n<h2 class=\"wp-block-heading\" id=\"h-&#x43A;&#x43B;&#x438;&#x43D;&#x438;&#x447;&#x435;&#x441;&#x43A;&#x43E;&#x435;-&#x437;&#x43D;&#x430;&#x447;&#x435;&#x43D;&#x438;&#x435;\"><strong>Klinische Signifikanz<\/strong><\/h2>\n\n<p class=\"wp-block-paragraph\">Das Auftreten eines einseitigen anhaltenden Mydriasis bei einem Patienten mit Sch\u00e4del-Hirn-Trauma ist ein klassisches Symptom, das mit mehr als 80 % Wahrscheinlichkeit auf das Vorhandensein eines massiven intrakraniellen H\u00e4matoms hinweist.<\/p>\n\n<p class=\"wp-block-paragraph\">Eine wichtige diagnostische Regel lautet: Die erweiterte Pupille tritt in der Regel auf der Seite der Blutung auf. Dies erm\u00f6glicht es dem Neurochirurgen, ohne Fehler zu erkennen, von welcher Seite der Zugang zum H\u00e4matom erfolgen soll, selbst wenn kein Computertomograph zur Verf\u00fcgung steht.
