Das Vorhandensein von freier Luft st\u00f6rt den physiologischen Unterdruck im Pleuraraum, was zum teilweisen oder vollst\u00e4ndigen Kollabieren des betroffenen Lungengewebes f\u00fchrt.<\/p>\n\n<h2 class=\"wp-block-heading\" id=\"h-&#x44D;&#x442;&#x438;&#x43E;&#x43B;&#x43E;&#x433;&#x438;&#x44F;-&#x438;-&#x43F;&#x430;&#x442;&#x43E;&#x444;&#x438;&#x437;&#x438;&#x43E;&#x43B;&#x43E;&#x433;&#x438;&#x44F;\"><strong>\u00c4tiologie und Pathophysiologie<\/strong><\/h2>\n\n<p>Normalerweise ist die Pleurah\u00f6hle vollst\u00e4ndig hermetisch und der Druck darin immer niedriger als der atmosph\u00e4rische. Bei geschlossenen oder offenen Brustkorbverletzungen wird dieses System undicht. In der Traumatologie ist dies die h\u00e4ufigste und lebensbedrohlichste Komplikation von Rippen- oder Schl\u00fcsselbeinfrakturen, wenn der scharfe Rand eines gebrochenen Knochens die Pleura durchbohrt.<\/p>\n\n<p>Luft str\u00f6mt in die Pleurah\u00f6hle und schlie\u00dft die Lunge von der Atmung aus. Am gef\u00e4hrlichsten ist der Spannungs- oder Ventilpneumothorax. Dabei wirken die besch\u00e4digten Weichteile wie ein Einwegventil: Luft str\u00f6mt bei jedem Einatmen in den Raum, kann aber beim Ausatmen nicht zur\u00fcckweichen. Dies f\u00fchrt zum Kollabieren der Lunge auf der gesch\u00e4digten Seite, dann zur Verlagerung des Mediastinums zur unbesch\u00e4digten Lunge mit anschlie\u00dfender Beeintr\u00e4chtigung ihrer Funktion. Eine schwere Komplikation ist das Umbiegen der Hohlvenen durch die Verlagerung des Herzens. <\/p>\n\n<h2 class=\"wp-block-heading\" id=\"h-&#x43A;&#x43B;&#x438;&#x43D;&#x438;&#x447;&#x435;&#x441;&#x43A;&#x43E;&#x435;-&#x437;&#x43D;&#x430;&#x447;&#x435;&#x43D;&#x438;&#x435;\"><strong>Klinische Signifikanz<\/strong><\/h2>\n\n<p>Die klinische Trias umfasst akuten, stechenden Brustschmerz, schwere Atemnot und erh\u00f6hte Herzfrequenz. Bei der Auskultation mit einem Stethoskop sind die Atemger\u00e4usche auf der betroffenen Seite stark abgeschw\u00e4cht oder vollst\u00e4ndig abwesend. Der Patient nimmt eine erzwungene sitzende Position ein und schnappt nach Luft.<\/p>\n\n<p>Die Diagnose wird durch eine \u00dcbersichtsaufnahme des Brustkorbs zuverl\u00e4ssig best\u00e4tigt.
