Unterhalb dieser H\u00f6he verlaufen im weiten Duralsack frei nur lange Nervenwurzeln (von L2 bis S5), deren Aussehen visuell an einen Pferdeschwanz erinnert.<\/p>\n\n<p>Das Syndrom entwickelt sich bei einer pl\u00f6tzlichen und kritischen Verengung des Lumens im Wirbelkanal auf H\u00f6he der Lumbalregion. Die h\u00e4ufigste Ursache ist ein massiver (sequestrierter) Bandscheibenvorfall, der direkt in das Lumen des Kanals f\u00e4llt und das gesamte Nervengeflecht gleichzeitig komprimiert. Seltener wird es durch eine Tr\u00fcmmerfraktur der Lendenwirbel oder ein ausgedehntes Epiduralh\u00e4matom verursacht.<\/p>\n\n<h2 class=\"wp-block-heading\" id=\"h-&#x43A;&#x43B;&#x438;&#x43D;&#x438;&#x447;&#x435;&#x441;&#x43A;&#x43E;&#x435;-&#x437;&#x43D;&#x430;&#x447;&#x435;&#x43D;&#x438;&#x435;\"><strong>Klinische Signifikanz<\/strong><\/h2>\n\n<p>In der Traumatologie und Neurochirurgie geh\u00f6rt diese Diagnose zu den absoluten \u201eRed Flags\u201c und erfordert eine sofortige Hospitalisierung. Das klinische Bild setzt sich aus einer spezifischen symmetrischen Triade von Symptomen zusammen.<\/p>\n\n<p>Das Hauptmerkmal ist die \u201eSattelan\u00e4sthesie\u201c \u2014 ein vollst\u00e4ndiger Verlust der Sensibilit\u00e4t im Perinealbereich, an der Innenseite der Oberschenkel und der Genitalien. Ein weiteres kritisches Symptom ist die erhebliche Dysfunktion der Beckenorgane: akuter Harnverhalt (der Patient sp\u00fcrt die Blasen\u00fcberf\u00fcllung nicht) und Stuhlinkontinenz aufgrund einer L\u00e4hmung der Schlie\u00dfmuskeln. Das dritte Merkmal ist ein schlaffer Paresis der unteren Extremit\u00e4ten mit Verlust der Achillessehnenreflexe. Der Zustand erfordert eine dringende Dekompressionsoperation innerhalb der ersten 24 Stunden, um eine irreversible Behinderung zu verhindern.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Das
