Sie sind die obligate Dominante (95-98 % der gesamten Bakterienmasse) der vaginalen Mikrobiota einer gesunden Frau im reproduktiven Alter. <\/p>\n\n<h2 class=\"wp-block-heading\" id=\"h-&#x44D;&#x442;&#x438;&#x43E;&#x43B;&#x43E;&#x433;&#x438;&#x44F;-&#x438;-&#x43F;&#x430;&#x442;&#x43E;&#x444;&#x438;&#x437;&#x438;&#x43E;&#x43B;&#x43E;&#x433;&#x438;&#x44F;\"><strong>\u00c4tiologie und Pathophysiologie<\/strong><\/h2>\n\n<p>Die h\u00e4ufigsten Arten im vaginalen Biotop sind <em>L. crispatus<\/em>, <em>L. jensenii<\/em>, <em>L. gasseri<\/em> und <em>L. iners<\/em>. Ihre physiologische Rolle besteht darin, durch drei Mechanismen eine Resistenz gegen die Kolonisierung zu schaffen: <\/p>\n\n<ol class=\"wp-block-list\">\n<li><strong>Acidogenese:<\/strong> Laktobazillen fermentieren das in den reifen Zellen des Vaginalepithels enthaltene Glykogen und bilden Milchs\u00e4ure. Dadurch wird der pH-Wert der Umgebung zwischen 3,8 und 4,5 gehalten, was ung\u00fcnstige Bedingungen f\u00fcr das Wachstum von pathogenen und opportunistischen Mikroorganismen schafft. <\/li>\n\n\n\n<li><strong>Synthese von Wasserstoffperoxid (H\u2082O\u2082):<\/strong> Viele St\u00e4mme produzieren H\u2082O\u2082, das in Gegenwart von Myeloperoxidase eine starke bakterizide Wirkung auf katalase-negative Anaerobier (z. B. <em>Gardnerella vaginalis<\/em>) hat.<\/li>\n\n\n\n<li><strong>Kompetitive Adh\u00e4sion:<\/strong> Bakterien bedecken dicht Rezeptoren auf der Oberfl\u00e4che von Epithelozyten, bilden einen sch\u00fctzenden Biofilm und verhindern physisch die Adh\u00e4sion von Pathogenen.<\/li>\n<\/ol>\n\n<h2 class=\"wp-block-heading\" id=\"h-&#x43A;&#x43B;&#x438;&#x43D;&#x438;&#x447;&#x435;&#x441;&#x43A;&#x43E;&#x435;-&#x437;&#x43D;&#x430;&#x447;&#x435;&#x43D;&#x438;&#x435;\"><strong>Klinische Bedeutung<\/strong><\/h2>\n\n<p>Ein verminderter Titer von Laktobazillen (<strong>weniger als 10\u2076 KBE\/ml<\/strong>) oder der Ersatz von H\u2082O\u2082-produzierenden St\u00e4mmen durch inaktive St\u00e4mme ist ein wichtiges Bindeglied in der Pathogenese der <strong>bakteriellen Vaginose<\/strong>, der aeroben Vaginitis und der Candidose. Bei der postmenopausalen <strong>atrophischen Vaginitis<\/strong> f\u00fchrt der \u00d6strogenmangel zur Einstellung der Epithelreifung und zum Verschwinden des Glykogens.
