Das aus dem Gef\u00e4\u00df ausgelaufene Blut bildet einen Hohlraum, der mit fl\u00fcssigem oder koaguliertem Blut gef\u00fcllt ist.<\/p>\n\n<p>Im Alltag werden kleine subkutane H\u00e4matome als Bluterg\u00fcsse oder blaue Flecken bezeichnet. Im medizinischen Sinne versteht man unter einem H\u00e4matom jedoch eine gr\u00f6\u00dfere Blutansammlung, die das umliegende Gewebe und die Organe komprimieren kann.<\/p>\n\n<h2 class=\"wp-block-heading\" id=\"h-&#x44D;&#x442;&#x438;&#x43E;&#x43B;&#x43E;&#x433;&#x438;&#x44F;-&#x438;-&#x43F;&#x430;&#x442;&#x43E;&#x444;&#x438;&#x437;&#x438;&#x43E;&#x43B;&#x43E;&#x433;&#x438;&#x44F;\"><strong>\u00c4tiologie und Pathophysiologie<\/strong><\/h2>\n\n<p>Die h\u00e4ufigste Ursache ist eine Verletzung (Prellung, Schlag, Fraktur), die zu einem Riss der Gef\u00e4\u00dfwand f\u00fchrt. Weniger h\u00e4ufig treten H\u00e4matome spontan im Zusammenhang mit Erkrankungen auf, die die Blutgerinnung beeintr\u00e4chtigen (H\u00e4mophilie) oder die Gef\u00e4\u00dfbr\u00fcchigkeit erh\u00f6hen.<\/p>\n\n<p>Das ausgetretene Blut gerinnt allm\u00e4hlich. Mit der Zeit durchl\u00e4uft das H\u00e4moglobin im H\u00e4matom eine Reihe biochemischer Umwandlungen, was die charakteristische Farbver\u00e4nderung von blau-violett zu gr\u00fcnlich und gelb (Verf\u00e4rbung des Blutergusses) erkl\u00e4rt. Im weiteren Verlauf bilden sich kleine H\u00e4matome vollst\u00e4ndig zur\u00fcck, w\u00e4hrend gr\u00f6\u00dfere H\u00e4matome sich verkapseln oder vereitern k\u00f6nnen.<\/p>\n\n<h2 class=\"wp-block-heading\" id=\"h-&#x43A;&#x43B;&#x438;&#x43D;&#x438;&#x447;&#x435;&#x441;&#x43A;&#x43E;&#x435;-&#x437;&#x43D;&#x430;&#x447;&#x435;&#x43D;&#x438;&#x435;\"><strong>Klinische Bedeutung<\/strong><\/h2>\n\n<p>Das klinische Erscheinungsbild h\u00e4ngt von der Gr\u00f6\u00dfe und Lage des H\u00e4matoms ab. Oberfl\u00e4chliche H\u00e4matome verursachen lokale Schmerzen, Schwellungen und Hautverf\u00e4rbungen. Tiefe H\u00e4matome, insbesondere intrakranielle, sind lebensbedrohlich, da sie das Gehirn komprimieren.<\/p>\n\n<p>Je nach Lokalisation unterscheidet man:<\/p>\n\n<ul class=\"wp-block-list\">\n<li><strong>Subkutanes und intramuskul\u00e4res H\u00e4matom:<\/strong> Das sind die h\u00e4ufigsten Typen.<\/li>\n\n\n\n<li><strong>Subdurales und epidurales H\u00e4matom:<\/strong> Das sind intrakranielle Blutansammlungen, die eine dringende neurochirurgische Behandlung erfordern.<\/li>\n\n\n\n<li><strong>Retrobulb\u00e4res H\u00e4matom:<\/strong> Es handelt sich um eine Blutansammlung hinter dem Augapfel.<\/li>\n<\/ul>\n\n<p>Die Diagnostik oberfl\u00e4chlicher H\u00e4matome ist unkompliziert. Zur Visualisierung tiefer und innerer H\u00e4matome werden Sonographie, CT oder MRT eingesetzt. Die Behandlung kleiner H\u00e4matome erfolgt konservativ (Ruhe, K\u00e4lte, Druckverband).
